I hereby contract with the owner that my pets will be in good health free from illness with any communicable condition/s within the last 14 days before Daycare each and every time I bring my pets. I further understand it is my responsibility to inform Country Comfort Kennels and its staff of any previous events in which my pets have climbed fences, destroyed fencing, harmed, shown aggressive or threatening behavior towards any person or any other dog.

 

• I understand that I am solely responsible for any harm caused by my dog(s) while my dog(s) is/are attending Daycare. 

 

• I further understand and agree that in admitting my dog(s) to Daycare, Country Comfort Kennel's Daycare staff have relied on my representation that my dog/s is/are in good health and has/have not shown aggressive or threatening behavior towards any person or any other dog.

 

• I further understand and agree that Country Comfort Kennels will exercise reasonable care and take all precautions necessary to keep all dogs safe and healthy, but I hereby waive and release Country Comfort Kennels and their staff, owners, and agents, from any and all liability, including but not limited to injury or damage, sickness, etc. and I expressly assume the risk of such damage or injury caused to or by my dog.

 

• I further understand and agree that in a Daycare setting there are some inherent risks. Even with diligent supervision, the unexpected can happen. In recognition of the benefits afforded my pet through participation in Daycare, I accept the potential risks. In the event of minor cuts and scrapes, I understand that dog(s) will be treated as deemed best by the staff of Country Comfort kennels at their sole discretion, and that I assume full financial responsibility for any and all expenses involved.

 

• I agree that I will not hold Country Comfort Kennels, staff, owners and agents liable for failure to seek veterinary attention for my dog.

 

• I understand that in the event that my dog bites a person while at Daycare at any of the Pennsylvania locations, the PA Bureau of Dog Law Enforcement MUST be notified. I agree to comply with all PA laws and pay any expenses associated with this compliance.

 

• In the event I must cancel my Day Care reservation with less than 24 hours of notice, I understand that I will be charged for a full day.

 

 

I certify that I have read and understood the policies of Country Comfort Kennels at Scott Creek Inc. as set forth and that I have read and accept the conditions and statements of this agreement.

 

I attest that I am authorized to act on behalf of any and all owners of dog(s) described herein.
